Date and time of next scheduled court date and whether this Tennessee court appearance is for General Sessions Court, a Tennessee Criminal Court, a Tennessee City or Municipal Court, or for a scheduled Tennessee booking (fingerprinted, photographed, etc.); and

Name of arresting Tennessee law enforcement officer, if known.
